"Confident Beginning": Effective Investments in Early Childhood
When it comes to business, many investors and entrepreneurs are ready to take risks in the hope of success in the future.
In charity, everything is somewhat different: the donors and society as a whole expect conservative decisions, and condemn risk and radical changes.
But many social tasks requires non -standard solutions.
One of these solutions, the confident beginning program, KPMG Russia experts told at the Doba-2020 conference, organized by the Good Mail.

Prerequisites and motivation An audit is a rather conservative business.
Coming to the market of corporate social responsibility (CSR), our team sought to find such a direction to help in which social investments would give maximum return.
We analyzed the entire line of services that is in the world, and realized that the segment of assistance in early childhood was the least developed.
At the same time, the famous Hekman curve shows that investment in early childhood is much more effective than all subsequent programs for a late age.

If we talk about diagnoses, then these are: cerebral palsy, malformations of the central nervous system, genetic syndromes, hearing and vision disorders, severe somatic diseases, epilepsy.
In a high -risk group there are also premature and small children.
Moreover, it is important that the whole family receive help, since the functionality of children is difficult to interconnected not only with the initial data of the children themselves, but also with the emotional state of parents and the situation in the family.
Decision “Confident beginning” is a program of early intervention in the format of a home visit to the family by specialists.
Implementation At first, KPMG Russia watched how such programs work in the USA.
It was there that more than 50 years ago the first programs of early intervention appeared.
Today they are integrated into the economy of all leading countries of the world and are a gold standard for helping vulnerable children.
For example, one of the programs of early Head Start intervention has more than 1 million children and an annual budget of more than $ 10 billion.
These services are financed by countries from institutional sources and are free for customers.
Starting the program in Russia, KPMG Russia decided to operate in partnership with reliable funds.
Three NPOs became partners: “communication space”, “Elizabethan Garden” and CLP.
KPMG Russia was important to integrate into the infrastructure of our country and its realities, and not create something artificial and unprofitable to life.
This is the key to the stability of the project: if one of the partners has problems, the rest will continue to implement the program, if it is important at the level of values, achieved by technology and has good chances to become profitable for the provider.
The program has four tricks of exposure: Development of functional skills in a child Prevention of secondary complications Improving the quality of life of the child Creation of a therapeutic alliance with a family The “confident beginning” program has a clear technology: with each child, experts work at his house, the development of each child is evaluated on certain scales, specialists, together with parents, set functional goals according to the SMART system for the next three months.
Functional – this means that we are talking about skills or other changes that are important and realistic for a particular child and his family.
Goals can concern any areas of child development.
Each visit is the work of a specialist, parents and child to achieve these goals.
Moreover, young children do not know the word “necessary”, so all the interaction with the child looks like a game, but each action in it is aimed at achieving the goals.
And the task of the specialist is not just to deal with a child from a paternalistic role, but literally a hand in his hand to convey these therapeutic strategies to parents.
Because if this is not there, then the “confident beginning” can be closed.
The intensity of the program is two visits per week, the remaining 70 hours that the child does not sleep, he and his family, and routines should become a place of development.
After each visit, the specialist should leave a “trace in the system” – fill out the documents.

The track of each child is conducted in standard forms from the first contact to release according to a certain structure.
At this stage, we were faced with the first difficulties: people who were accustomed to work in the emotional sphere could not join it for a long time.
So the workflow at the initial stages occupied many more than 30% of the time.
It took people to teach and accustom people to set quality goals using the Smart system for a year and a half.
And the point here is not that people did not want – it’s just a really difficult skill, taking into account the specifics of our little customers and the modern approach in the field of early intervention.
results We built a full -fledged high -quality and affordable service in Moscow.
The “confident beginning” uses only methods with proven effectiveness.
Today it is the biggest program of early intervention in home visitor format in the country.
Since the launch of the program, more than 400 children have become its clients, with a simultaneous capacity of 90 children.
Children are in the program until 36 months.
Two years ago, in the program, the level of achievement of individual goals was 65%, now this figure has reached 75%.
This means that the functionality of children has grown significantly.
We know for sure that not a single child for this period was given to the boarding school.
Over the indicated time, no child had the most severe and common complication in the program – a dislocation of the hip joint.
We also see that more than half of the parents decreases the level of stress.
However, this indicator is nonlinear.
Often, with an increase in the client position, parents better understand the dramatic lag of our country in the field of systemic assistance, inclusion and rehabilitation.
It rather enhances stress.
We also see a reliable increase in support level in 75% of parents.
With the growth of the partner position and cooperation strategies, all this is used by families outside the program.
At the same time, the program is not suitable for everyone.
At the beginning of about 10% of customers, a decision on early exit was made, now there are less than 7% of such families.
With each such family at the exit, we make a final interview to study the causes and situation as a whole.
The interview is done by another service to exclude a conflict of interest.
The total cost of investment at the end of the fourth year of work will reach 85 million rubles.
But it is worth noting the economic benefit of these investments for donors, one child’s annual contract in the confident beginning program costs about 240 thousand rubles, despite the fact that the average price of course rehabilitation for three to four weeks costs about 200 thousand rubles.
However, the effect (investment return) is a parallel improvement of the functionality of children and the emotional state of their families.
Parents of children in the program will rather continue to be implemented in the profession, preserve and develop social ties, as well as think about the birth of subsequent children.
Plans KPMG Russia is interested in replicating the confident beginning program.
Today, even in Moscow, we close only about 6.
5% of the need market.
In the country, according to the Ministry of Labor, about 407 thousand children need early intervention programs.
These children are anywhere in the country, in the areas of the presence of any business.
But without solving the issue with additional investments, we alone cannot be solved by this problem.
In the framework of the “confident beginning” in the coming years, we want to launch a multifactor study of effectiveness, there is no internal research in this area in Russia.
And they are needed, including as important arguments for stakeholders.
We will also continue to work on clinical and managerial quality, the creation of a professional community, integration with colleagues from other countries.
